Stephen F. Austin Community Health Network (SFACHN)
Stephen F. Austin Community Health Network (SFACHN) was established in 2008 to provide accessible and affordable healthcare for people of Brazoria County. That year, the Center had one health care provider with three exam rooms providing care at the Adoue Family Health Center. Since that time, the Center has grown substantially, adding a dental department, women’s health care, behavioral health counselors, and psychiatry. In 2013, SFACHN launched a second site located in Freeport, Texas. The Freeport Community Health Center provides adult medicine, women’s health, obstetrics & gynecology, behavioral health and general dentistry services.
Due to growth and demand for services in Alvin, SFACHN has opened two additional sites: the Women and Children’s Health Center and SFACHN Administration where we provide psychiatry, group & individual counseling services. The Center’s newest location is our Stephen F. Austin Dental Care Associates located at 1506 E Broadway Street which opened in May of 2017. In the Center’s first year of service 1,025 patients received care, In 2016 the number of people that received care was 11,923.
