A Discussion of Medicaid’s $5 Billion/Year IT Infrastructure Transformation
CMS provides funding to the tune of 5 billion dollars per year to support the Medicaid information technology platforms run by the states. In December 2015, CMS issued a final rule, Mechanized Claims Processing and Information Retrieval Systems (90% Federal/10% State), to assist states to update Medicaid Management Information Systems (MMIS) in over 20 states. These changes will allow states to improve customer service and support the dynamic nature of Medicaid eligibility, enrollment, and delivery systems. Also within this rule was language directing the Medicaid Enterprise towards an open, modular architecture.

Defining An Open Platform for Health IT
It is widely agreed that the future of digital health lies in an “Open Platform”. However, it’s not clear as to exactly what an Open Platform is or how we get there. This blog aims to answer the first question and to provide some guidance on the second. While any given instance of an Open Platform will be a specific implementation of a set of software components owned and operated by a particular organisation (this might be a health and social care organisation or a third party, operating the platform on behalf of a local health and care community), it is most usefully defined by a set of principles rather than the specific details of a particular implementation.

Open Letter to Chuck Hagel: DoD still doesn’t know what the hell they are doing
...I fear that you are paving a road to a hellish destination. Rather than lifting up the VA eligibility problem to a shiny new common information system, you are on the verge of dragging health IT into the same bureaucratic vortex that has already done so much damage in the past. AHLTA was declared “intolerable” in a Congressional hearing 4 years ago.... Read More »

Open Source Governance and the Rise of a New Open Health Movement
It's hard to tell if (or when) new open source foundations will appear and claim a leading role in healthcare. It would be interesting to see one created to scale an existing viable model, such as the one from Oroville Hospital using VistA. Or we could see OSEHRA shifting its focus and expanding its charter beyond just the US government space. Nevertheless, the successful foundation would keep a low barrier to entry for innovators, allowing them to incorporate and scale open source healthcare technologies into commercial products. Time will tell, but what's for certain is that we live in interesting times, and I am looking forward to massive innovation in healthcare in the near future. The time is ripe.

Overview of Open Source and VistA in the UK's NHS
There is much widely publicised interest from NHS England in encouraging the development and implementation of open-source software in the National Health System (NHS) with the debate raging in a number of forums, notably on EHI where this article and the comments it has generated are vital reading for anyone interested in this issue. This debate has been fueled by the availability of NHS England’s £260 million Technology Fund which is actively soliciting open source projects include bids to implement an NHS VistA... Read More »

Pentagon Resists Administration’s Mandate for an Open Source Health Records System
President Obama has backed open standards for an integrated electronic health record system to serve the Defense and Veterans Affairs departments since his first term, but Pentagon plans to acquire commercial software to replace the department’s current EHR are “manifestly inconsistent” with that approach, J. Michael Gilmore, the Pentagon’s director of operational test and evaluation wrote in a blistering memo. Read More »

The Drupal API turns a CMS into a True Health Care Enterprise Application
At the company where I work, Achieve Internet, we view Drupal as more than just a CMS — we see it as a powerful web application platform with capabilities to integrate multiple sources of information. Sporting a far-reaching and flexible API, Drupal can link together other platforms that provide APIs, such as enterprise productivity systems or electronic health records (EHRs), and essentially provide Drupal’s web pages as an interface to these systems on both a read and write basis. The growth of the platform and the community has put Drupal in a position to revolutionize the concept of a traditional CMS in one market sector after another, from the media and entertainment industries to education, travel, and government.
