On January 18, 2022 the Office of the National Coordinator for Health Information Technology (ONC) finally released version 1 of the Trusted Exchange Framework, Common Agreement and the QHIN (Qualified Health Information Network) Technical Framework (QTF). Several years in the making, these documents represent the latest attempt at initiating a national health information exchange in the United States. This project is being managed by ONC’s Recognized Coordinating Entity (RCE), The Sequoia Project, and was inspired by both the HITECH Act and 21st Century Cures Act.

A Public Health Perspective on ONC's Strategy to Reduce Burden on Physicians
On November 28, 2018, the Office of the National Coordinator for Health Information Technology (ONC) released a draft Strategy on Reducing Regulatory and Administrative Burden Relating to the Use of Health IT and EHRs for public comment. The strategy aims to reduce the time and effort and improve the functionality of electronic health records (EHRs) for clinicians, hospitals, and other healthcare organizations. This strategy was developed primarily through the efforts of ONC-convened workgroups in response to requirements laid out by Congress in the 21st Century Cures Act (Section 13103). The report itself does not identify who exactly served on these workgroups and what organizations were represented.

CMS Releases Proposed Updates to Medicare MIPS Promoting Interoperability Program for Physicians
On July 29, 2022 the Centers for Medicare and Medicaid Services (CMS) released a notice of proposed rulemaking (NPRM) related to changes in the Medicare Program Physician Fee Schedule for 2013. Among the proposals in this lengthy document are those related to the Promoting Interoperability Program for physician practices, the successor to the Meaningful Use of Electronic Health Record (EHR) technology that was originally rooted in the 2009 HITECH Act. This program has been evolving over the years and this NPRM proposes some meaningful changes to the public health reporting component which would first be used for calendar year 2023.

Grant Supports Development Of Software To Judge Quality Of Electronic Public Health Data
With the growing need for early identification of emerging threats including those of bioterrorism, pandemic flu, Ebola and foodborne illnesses, public health departments nationwide are increasingly relying upon data captured from electronic sources. A $381,000, 2-year grant from the National Library of Medicine at the National Institutes of Health supports development by the Regenstrief Institute and the Indiana University Richard M. Fairbanks School of Public Health of open source software tools to measure and monitor the quality of electronic data being transmitted to public health departments across the nation from health care systems, medical laboratories, physician offices and other sources.

Halamka on Why he Disagrees with the "Snake Oil" Analogy

As a provider and a technologist, I think we need a deeper dive to understand the issues, avoiding the kind of hyperbole that’s so common in politics today. Paul B. HL7 Publishes First Domain Analysis Model for Clinical Sequencing

HLN to Present Innovative Open Source Solutions at HIMSS19 Interoperability Showcase
For the third straight year, HLN Consulting, a leading medical informatics consulting company, will participate in the Interoperability Showcase at this year's HIMSS19 Global Conference and Exhibition which is being held in Orlando, FL from February 11-15, 2019. Not All Snake Oil Is Digital
A different take on "snake oil" in health care was a thoughtful piece in Health Affairs, by David Newman and Amanda Frost, discussing the quality measurement morass in health care. They cite a study that estimated we spend some $15.4b annually collecting several thousand different quality measures, few of which have any meaning to consumers and all-too-few of which seem to be used to actively improve quality. It isn't that they don't think we should be measuring quality -- far from it -- but, rather: "Patients should not be able to choose substandard quality care, and substandard quality care should not be allowed to be offered in the market." Now, there's a novel concept!

ONC Finally Releases TEFCA—What it Might Mean to Public Health
ONC's Trusted Exchange—A Public Health Perspective
In January 2018 the Office of the National Coordinator for Health Information Technology (ONC) issued a draft Trusted Exchange Framework and Common Agreement (TEFCA), and related supporting documents, in response to a requirement imposed by Congress in the 21st Century Cures Act. The Act says that the TEF may include a common method for authenticating users, a common set of rules, enabling policies, and a process for managing non-compliance. Nowhere does the Act instruct ONC to determine an actual technical architecture in this process, though such a step is not precluded either. The primary document is in two parts: Part 1 is a set of principles that set the foundation for Part 2 which is a set if minimum terms and conditions for trusted exchange.

Open Source Machine Learning Tools are as Good as Humans' in Cancer Surveillance According to Regenstrief, Indiana Univ. Study
Machine learning has come of age in public health reporting according to researchers from the Regenstrief Institute and Indiana University School of Informatics and Computing at Indiana University-Purdue University Indianapolis. They have found that existing algorithms and open source machine learning tools were as good as, or better than, human reviewers in detecting cancer cases using data from free-text pathology reports. The computerized approach was also faster and less resource intensive in comparison to human counterparts.

Health reform is taking off, thanks to pressure from insurers, the promise with which innovative technologies tease us for low-cost treatments, and regulatory mandates dating back to the HITECH act of 2009.
